Report on fatal opal mine incident released

The NSW Resources Regulator has released an investigation report and incident animation video following the death of a miner in October 2020.

The incident occurred at an opal mine near Lightning Ridge.

A team of investigators from the NSW Resources Regulator found that the mine had not installed an appropriate working at heights system at the time of the incident.

The report describes the investigation, the findings and recommendations to the mining industry.
