Fatality at Queensland quarry triggers safety warning

The Queensland safety regulator for mining and quarrying has issued a safety alert after the death of a forklift operator at a quarry last month.

Resources Safety and Health Queensland (RSHQ) said it is probing the incident that happened on Monday, 20 April.

RSHQ Operations General Manager Donna Heelan stated preliminary information suggested the worker was crushed when his forklift toppled onto its side while moving on a ramp.

“Our team is on site and are conducting a thorough investigation,” she said.

“The purpose of the Safety Alert is to provide industry with initial advice to review the mitigation and management of risks associated with forklift operation to keep all workers as safe as possible.”
