Tasmania introduces new workers compensation claim forms and medical certificate

The Tasmanian Government says it is committed to ensuring workers have timely access to workers compensation after the State Government recently unveiled a set of new claim forms.

These documents include a Workers Compensation Claim Form, a Dependants of Deceased Workers Claim Form, and Workers Compensation Certificate of Capacity (medical certificate).

Minister for Workplace Safety and Consumer Affairs, Madeleine Ogilvie said the Government consulted widely before developing the forms and certificate.

“These forms are vitally important for ensuring Tasmanians get the workers compensation system they deserve by collecting the information needed to support a timely and effective workers compensation claim process,” Minister Ogilvie said.
